What's in the template
| Version | What it asks |
|---|---|
| Restaurant | Recommendation, food, service, wait time and comments |
| Coffee shop | Recommendation, drink, speed, visit frequency and what they'd like to see |
| Auto shop or dealership | Recommendation, whether the problem was fixed, on-time delivery, the advisor's explanation and comments |
| Beauty salon or spa | Recommendation, result, punctuality, whether they'd book again and what to improve |
| Store | Recommendation, whether they found what they wanted, service, checkout wait and products they'd like |
They all start with the same question: "From 0 to 10, how likely are you to recommend us?" That's the question behind NPS, the number most businesses use to track satisfaction month to month. Work it out with the free NPS calculator.

How to use it so people answer
- Hand it out at the end, with the bill or when you return the vehicle, when the customer has a minute.
- Ask them to fill it in, don't leave it optional. "Could you help us with these five questions?" works much better.
- Have a box or a person collect it. If it's left on the table, it gets lost.
- Read every answer daily and call anyone who scored 6 or below. That customer can still be won back.
- Count the answers every week to see whether you're improving.
The problem with paper surveys
The template gets you started today, but paper has three limits you'll notice fast:
- You find out late. If a customer rates you 2 on Friday night, you read it on Monday, after they've told their friends.
- Someone has to type it all into Excel to get averages and compare locations, and it rarely happens.
- Fewer people answer. Many would rather scan a code with their phone than look for a pen.
The same survey, digital and with alerts
With SnapRate these same questions become a survey customers answer on their phone, with nothing to download:
- One QR code per table, location or register, and every response arrives tagged with where it came from.
- A WhatsApp alert to the manager the moment someone rates you poorly, so you can call them the same day.
- A dashboard with your NPS and satisfaction per location, day by day, and reports that arrive by email on their own.
- Phone surveys for your after-sales team, if you run an auto shop or dealership.
- And if you want, after the survey the customer joins your loyalty program with a card in Apple Wallet or Google Wallet.
